В настоящее время нет единой концепции относительно эффективности применения флуоресцентной микроскопии (ФМ) у пациентов с новообразованиями, расположенными вблизи функционально значимых зон головного мозга (ФЗЗ), где резекция может быть осознанно остановлена хирургом ввиду высокого риска нарастания неврологического дефицита.</p> <p><bold>Задача.</bold> Оценить эффективность ФМ при удалении глиом вблизи моторной коры (МК) и кортикоспинального тракта (КСТ).</p> <p><bold>Методика исследования.</bold> 108 взрослых пациентов с глиомами вблизи МК и КСТ, оперированных с применением нейрофизиологического мониторинга, разделены на две группы, в зависимости от использования ФМ: контрольную (34 пациента без ФМ) и основную (62 пациента с ФМ).</p> <p><bold>Результаты.</bold> Объем резекции, частота неврологического дефицита в послеоперационном периоде статистически значимо не отличались в исследуемых группах.</p> <p><bold>Заключение.</bold> Несмотря на отсутствие статистически значимой разницы между группами, применение ФМ может быть полезным в составе комплексного интраоперационного мониторинга у данной группы пациентов.</p></trans-abstract><kwd-group xml:lang="en"><kwd>fluorescence-guided surgery</kwd><kwd>intraoperative fluorescence</kwd><kwd>neurophysiological monitoring</kwd><kwd>corticospinal tract</kwd><kwd>motor cortex</kwd><kwd>gliomas</kwd><kwd>5-aminolevulenic acid</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>флуоресцентная микроскопия</kwd><kwd>интраоперационная флуоресценция</kwd><kwd>нейрофизиологический мониторинг</kwd><kwd>кортикоспинальный тракт</kwd><kwd>моторная кора</kwd><kwd>глиомы</kwd><kwd>5-аминолевуленовая кислота</kwd></kwd-group><funding-group><award-group><funding-source><institution-wrap><institution xml:lang="ru">Министерство науки и высшего образования Российской Федерации</institution></institution-wrap><institution-wrap><institution xml:lang="en">Ministry of Science and Higher Education of the Russian Federation</institution></institution-wrap></funding-source><award-id>075–15–2021–1343</award-id></award-group></funding-group></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Косырькова А.
