The patients were randomized into two groups. In the main group (43 patients), the original RME method (RU patent No. 2748639, dated May 28, 2021) was used; in the control group (39 patients), the standard RME method was applied.</p> <p><bold>Results. </bold>The use of a standard elliptical incision for RME in obese patients leads to the formation of lateral conical defects, the frequency of which reaches 74%. There is a high correlation between the frequency of formation of lateral conical defects in radical mastectomy with expansion of the breast base (Spearman index 0.85 (p≤0.05), chest circumference (Spearman index 0.92(p≤0.05), fat pedicle thickness (Spearman index 0.94 (p ≤ 0.05) and degree of mammoptosis (Spearman index 0.98 (p ≤ 0.05).</p></abstract><trans-abstract xml:lang="ru"><p><bold>Цель</bold> – установить основные причины формирования боковых конических дефектов у пациенток с ожирением после радикальной мастэктомии по поводу рака молочной железы.</p> <p><bold>Материал и методы. </bold>Проведен проспективный анализ 82 больных раком молочной железы с ожирением различной степени. Больные рандомизированы на две группы. В основной (43 пациентки) применен оригинальный способ РМЭ (патент RU № 2748639 от 28.05.2021), в контрольной – традиционный способ РМЭ (39 пациенток).</p> <p><bold>Результаты.</bold> Применение стандартного эллипсовидного разреза при радикальной мастэктомии у пациенток с ожирением приводит к формированию боковых конических дефектов, частота которых достигает 74%. Наблюдается высокая корреляционная связь между частотой формирования боковых конических дефектов при радикальной мастэктомии и расширением базы молочной железы (индекс Спирмена 0,85, p≤0,05), окружностью грудной клетки (индекс Спирмена 0,92, p≤0,05), толщиной жировой ножки (индекс Спирмена 0,94, p≤0,05), степенью маммоптоза (индекс Спирмена 0,98, p≤0,05).</p></trans-abstract><kwd-group xml:lang="en"><kwd>radical mastectomy</kwd><kwd>breast cancer</kwd><kwd>lateral conical defect</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>радикальная мастэктомия</kwd><kwd>рак молочной железы</kwd><kwd>боковой конический дефект</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Bawoke G, Segni K, Abebe A, Girmaye T.
