Magnetic resonance imaging is commonly used to evaluate the TMJ due to the high contrast resolution and the ability to obtain dynamic images to demonstrate the functional capability of the joint. Aim - to determine the functional changes after TMJ splint therapy conducted at various stages of intra-articular disorders. Data and methods. We conducted a survey, clinical examination and MRI of TMJ of 37 patients with temporomandibular disorders (TMD). Two groups of patients were formed according to the received MR picture of TMJ. Results. The study highlighted that regression of pain in TMJ and masticatory muscles, and normalization of functional parameters after splint rehabilitation occur more often in the first group of patients, who have disorders of the articular disc of the TMJ. Conclusion. In both groups of patients we noted positive dynamics of functional parameters of TMJ, reduction of pain in TMJ and masticatory muscles. Though 36,8% of the second group of patients, who have bone remodeling of TMJ condyles, didn’t show dynamics of functional parameters. Consequently, the use of splint in one third of patients with temporomandibular joint condyles destruction is effective for TMJ and muscular pain reduction.</p></abstract><trans-abstract xml:lang="ru"><p>Визуализация височно-нижнечелюстного сустава (ВНЧС) постоянно развивается с усовершенствованием технологий. Магнитно-резонансная томография обычно используется для оценки ВНЧС благодаря высокому контрастному разрешению и способности получать динамические изображения для демонстрации функциональных возможностей сустава. Цель - определение функциональных изменений ВНЧС после проведенной сплинт-терапии при различных стадиях внутрисуставных нарушений. Материалы и методы. Было проведено анкетирование, клиническое обследование, МРТ височно-нижнечелюстных суставов 37 пациентов с краниомандибулярными дисфункциями. Сформировано 2 группы пациентов в соответствии с полученной MP-картиной ВНЧС. Результаты. Исследование показало, что регрессия болевого синдрома в области ВНЧС и жевательных мышц, нормализация функциональных параметров после проведенной реабилитации на сплинтах происходит чаще у пациентов первой группы, имеющих нарушения на уровне суставного диска ВНЧС. Заключение. В обеих группах пациентов отмечается положительная динамика функциональных параметров ВНЧС, редукция болевого синдрома в области ВНЧС и жевательных мышцах, однако у 36,8% пациентов 2-ой группы, имеющих костное ремоделирование головок ВНЧС, не наблюдается динамика функциональных параметров.
