STRATIFICATION OF PREGNANT WOMEN AT EARLY GESTATIONAL AGES BY MEANS OF OBJECTIVATION OF "PHYSIOLOGICAL ALTERATION" FACTORS, MECHANISMS OF GESTATIONAL ADAPTATION AND FETOPLACENTAL DYSFUNCTION

Abstract

Aim - allocation of clinical and pathogenetic variants of early gestosis in pregnant women based on clinical manifestations and markers of leading pathogenetic syndromes of gestational maladjustment. Materials and methods. 45 healthy pregnant women and 160 pregnant women with early morning sickness of varying severity were examined in the 1st trimester of gestation. The comparison group consisted of 33 healthy non-pregnant women. The survey included assessment of clinical symptoms, identification of the markers of inflammatory response, energy balance of reproductive system, functional activity of the endometrium, vascular endothelial and hemostatic disorders, and blood biochemical indices. Results. The study enabled quantitative objectivation of the factors of “physiological damage” and “the mechanisms of gestational adaptation” during physiological gestation, defining fetoplacental dysfunction as the key link of maladjustment in early pregnancy. The prevalence of pathogenetic mechanisms of the formation of early pregnancy toxicosis determined the allocation of 4 clinical-pathogenetic variants of the course of this complication of gestation. Conclusion. Differentiated approach to clinical and laboratory manifestations of early gestosis in pregnant women, taking into account the variants of the clinical course of this complication and severity assessment, enables targeted therapy and predicting the risk of development of severe forms and late obstetric complications, reduces the risk of adverse perinatal outcomes.

METHOD OF GRAVITATIONAL THERAPY IN TREATMENT OF GASTROINTESTINAL TRACT DISORDERS

Abstract

Aim - to increase the efficiency of treatment of patients with gastrointestinal tract disorders by including the method of gravitational therapy into the treatment complex. Methods. The method of gravitational therapy was used to achieve beneficial impact on the diseases of the gastrointestinal tract. Results. The paper presents the relevance of the treatment of gastrointestinal tract diseases and the search for new treatment options that have fewer side effects and reduce the cost of treatment. It also explores the influence of gravitational therapy on diseases of the gastrointestinal tract, and further introduction of this method in the treatment regimens. It specifies the importance of complementing medical treatment of gastrointestinal tract diseases with the physiotherapy methods. We present indications for diseases of the digestive system and contraindications for gravitational therapy. Conclusion.Positive influence of gravitational therapy in cases of gastrointestinal tract disorders was revealed.

GENETIC POLYMORPHISMS OF CYTOCHROME P450 2C9, VITAMIN K EPOXIDE-REDUCTASE SUBUNIT 1 AND WARFARIN DOSING IN PATIENTS WITH PERMANENT ATRIAL FIBRILLATION

Abstract

Aim - to assess the influence of genetic polymorphisms of CYP2C9 and VKORC1 on warfarin dosing in patients with permanent form of atrial fibrillation. Methods. The study included examination of 100 patients with coronary heart disease and permanent form of atrial fibrillation; mean age 60.5±5.8 years. Conclusion. In patients with permanent form of atrial fibrillation, genotypes CYP2C9*1/*3, TT and GG genotypes of VKORC1 are associated with low warfarin dose, compared to other genotypes. Genotype CYP2C9*1/*3 increases the risk of bleeding complication during warfarin therapy.

SYSTEM OF MEASURES FOR REPRODUCTIVE HEALTH IMPROVEMENT IN WOMEN OF LATE REPRODUCTIVE AGE WITH INFERTILITY

Abstract

Aim - to develop and evaluate the effectiveness of the system of the medical and organizational measures aimed at improvement of reproductive health in women of late reproductive age with infertility, and at increase in the effectiveness of the IVF programs. Materials and methods. The study of the efficiency of the developed system of measures was carried out in a representative group (650) of women 35-44 years old (mean age - 37.01+ 1.74 years) with infertility, living in the Samara region and referred to the medical center «Dynasty» for treatment with in vitro fertilization. The following methods of research were used: direct observation, organizational experiment, sociological and analytical methods. Results. The main components of the developed system of events are: medical measures (extended timely examination and treatment; comprehensive assessment of women's reproductive health based on proposed medical criteria, the use of new medical technologies); social measures (individual program of social and hygienic rehabilitation); organizational measures (automation of infertility register control, routing of patients depending on the initial state of health). Implementation of the developed system of measures can improve women's reproductive health according to the criteria of objective and subjective evaluation: it reduces the incidence of miscarriage from 37.5% to 15.3% (p <0.05), increases the live birth rate 5 times (one in three women (29.1%) using the developed recommendations had a baby, and in the group of women who did not consider it necessary to carry out the recommendations submitted, only 5.8% (p <0.05) of the women gave birth to a child). Conclusion. The developed system of medical and organizational measures for women of late reproductive age with infertility proved to be effective in specialized medical care with the use of IVF technologies, and it is recommended for usage to increase the effectiveness of treatment of women of this age group with infertility.

THE PECULIARITIES OF THE IMMUNOLOGICAL DISORDERS AT VIBRATION DISEASE

Abstract

Aim - to study the state of cellular immunity and cytokine profile at vibration disease due to exposure to local and general vibration, taking into account the severity of the disease. Assessing the role of immunological changes in the pathogenesis of vibration disease. Material and methods. The study involved 84 patients with various forms and severity of vibration disease due to exposure to local and general vibration. The control group included 30 people who did not have contact with occupational hazards at work and who were recognized as healthy according to a complete physical examination. Diagnosis was based on occupational history, sanitary characteristics of working conditions (work in contact with local or general vibration, the vibration levels excessing maximum acceptable level for more than 15 years), full clinical functional screening and instrumental examination of patients. Methods of immunological study included determination of subpopulations of lymphocytes, levels of cytokines IL-ф, IL-4, IL-8, tumor necrosis factor alpha (TNF-а) in serum by ELISA. Statistical processing of the data was performed using SPSS software package 21. Results. The study revealed a significant decrease in helper (CD4+) and suppressor (CD8+) subpopulations of lymphocytes, increase of concentration of pro-inflammatory cytokines IL-ф, IL-8, TNF-а; reduction in anti-inflammatory cytokine IL-4 in patients with vibration disease compared with the control group, regardless of the type of vibration, but more pronounced at the second degree of vibration disease due to general vibration. Revealed changes are indicative of the activation of inflammatory processes, violation of immunological reactivity, the presence of cytokine imbalance as the effect of exposure to occupational vibration. Conclusion. The study of the immunological profile (cellular link, cytokine profile) at vibration disease opens up prospects for rehabilitation and treatment of patients with this pathology, and also elaborates our knowledge of the pathogenesis of this disease.

ANALYSIS OF INFLUENCE OF RUTIN AND GRAVITY ACTION ON THE RENAL EXCRETORY FUNCTION

Abstract

Aim - to analyze the effect of rutin on the renal excretory function in the conditions of normogravity and hypergravity. Materials and methods. 9 series of experiments were conducted, including 4 series with the use of a centrifuge of ultra-short radius. The effect of rutin in the dose of 10 mg/kg on the renal excretory function was studied in normal conditions and in combination with the gravity action 3g. All the animals received 3% water load intragastrically and were placed into exchange cages for 24 hours. We obtained urine samples and determined the amount and the concentration of sodium, potassium and creatinine. Results. It was found that under normal conditions rutin, administered intragastrically in a dose of 10 mg/kg, promotes significant increase in diuresis and saluresis solely due to the inhibition of tubular reabsorption. Rutin in a dose of 10 mg/kg in combination with the gravity action enhances its original diuretic activity by complex influence on tubular and glomerular kidney structures. Rutin alone and rutin in combination with hypergravity stimulates the excretory function of the kidneys similarly to hydrochlorothiazide, but has a shorter latent period. Conclusion. The use of rutin combined with the gravity action contributes to the stimulation of the renal excretory function not only due to the tubular component, but also due to the glomerular one.

THE DEVELOPMENT OF THE COMBINED MEDICINAL HERBAL REMEDIES FOR STOMATOLOGICAL AND OTORHINOLARYNGOLOGICAL PRACTICE

Abstract

Aim - to develop a series of combined medicinal herbal remedies «Dentos» for dental and otorhinolaryngological practice, in which complex effect on different elements of the infectious inflammatory process is achieved by the combination of several groups of biologically active compounds. Materials and methods. Basic phytocomposition is a medicinal preparation "Dentos", which is composite alcohol-water extract on 70% ethanol from the five types of medicinal plant raw materials: the leaves of Eucalyptus viminalis, the herb of Echinacea purpurea, the flowers of Calendula officinalis, the bark of Quercus robur and the oil of Caryophyllus aromaticus. Dosage forms based on it are the following: dental gel, medicinal lozenges, therapeutic and prophylactic toothpaste. The experimental research involved the methods of marketing analysis, various technological methods (extraction methods of developing medicinal preparations and instrumental definition of technological parameters), and modern methods of pharmaceutical analysis: chromatographic methods (TLC), UV-spectroscopy, HPLC, chemical methods (qualitative test-tube reaction). Results. The blend composition of the developed medicinal preparations was proposed (dental gel, medicinal lozenges, therapeutic and prophylactic toothpaste), the technology of their production was elaborated, the quality parameters were studied and the approaches to standardization of remedies were substantiated.

COMPOSITION OF ESSENTIAL OIL OF TANACETUM VULGARE L. FROM SAMARA REGION

Abstract

Aim - to analyze the essential oil isolated from the tansy (Tanacetum vulgare L.) flowers harvested from the spontaneous flora of Samara region. Materials and methods. The analysis was made on head flowers of Tanacetum vulgare harvested from the spontaneous flora of Samara region during the flowering stage (July), in 2016. The isolation of the essential oil was made in accordance with the Pharmacopeia of the Russian Federation XIIIth Edition. The essential oil was analyzed using a gas chromatograph coupled with mass-spectrometer apparatus. GCMS analysis was performed using an Agilent Technologies 7820F gas-chromatograph, series MSD, Agilent 5975. Results. It was determined that the major compounds of essential oil are represented by 2 isomers of thujone, eucalyptole (1,8-cineole) and yomogi-alcohol. According to the results of headspace analysis, the main identified components were artemisia alcohol, sabinene, eucalyptole (1,8-cineole). Conclusion. The knowledge of phytochemical characteristics of Tanacetum vulgare L. is important for the correct identification of the plant material for medical purposes.

FEATURES OF THE COLON MUCOSA IN PATIENTS WITH EXACERBATION OF ULCERATIVE COLITIS

Abstract

Aim - to study the features of the colon mucosa in patients with exacerbation of ulcerative colitis. Material and methods. Methods of general morphology and morphometry were developed and applied during the examination and treatment of 185 patients with exacerbation of ulcerative colitis in the clinic of hospital surgery from 2004 to 2015. Results. The efficacy of endolymphatic antibacterial and immunostimulating therapy was revealed. When comparing I and II groups of patients with moderate form of the ulcerative colitis, the difference between such indicators as the condition of the epithelium of the crypts and the depth of the lesion was statistically significant (p<0.05). Conclusion. The use of methods of general morphology and morphometry allows us to give an objective assessment of the severity of ulcerative colitis and to evaluate the dynamics of the histological structure of the colon wall during the treatment process.

EXTERNAL RADIO-FREQUENCY DENERVATION OF RENAL ARTERIES

Abstract

Aim - study of renal arteries denervation for the surgical treatment of primary arterial hypertension. Materials and methods. Radio-frequency ablation from the side of artery adventitia was carried out on preparations of the renal arteries. Results. The study determined comprehensible regimes of radio-frequency ablation, not resulting in a large degradation of the renal arteries strength properties, but providing reliable denervation of these arteries. Micro-photos of the renal artery wall after carrying out radio-frequency ablation were obtained. Conclusion. It is shown that the most comprehensible way of the renal arteries denervation is radio-frequency ablation from the side of artery adventitia.
